At Love Family AFC, our mission is to empower families and caregivers with the tools, resources, and compassionate support needed to provide exceptional care for their loved ones. We are dedicated to enhancing the quality of life for our community members by fostering independence, dignity, and a sense of belonging through personalized care and unwavering support.
Our Story
Love Family AFC is built on a foundation of care, compassion, and community. Our journey began at Dia Feliz ADHC, a community-based program that provided nursing care, supervision, and vital health-related services for individuals with physical, cognitive, or behavioral health impairments. It was here that we saw firsthand the profound impact of creating a supportive environment, not only for participants but for their families and caregivers as well.
Inspired by the warmth, resilience, and strength of the Dia Feliz ADHC community, we recognized the need to bring this same level of care and connection into the homes of families across our community. With a deep understanding of the struggles families face in managing loved ones’ care, we set out to create a program that could provide more than just services—it could provide hope, confidence, and a sense of family.
Today, Love Family AFC carries forward the spirit of Dia Feliz ADHC. We are committed to guiding families through the complexities of caregiving while ensuring their loved ones can live fulfilling, independent lives in the comfort of their own homes.
